We are proud to assist Hospice staff with any form of care that they are unable to provide due to a long-term illness or respite care not in the facility. Many family members choose to stay at home with private duty home health agency assistance. They provide our staff and the client with many specialized needs. With the 24-hour nurse provided by Hospice, A Caring Companion Home Health LLC will develop a care plan along with the family members and doctors to assist our client with their daily needs. We provide nursing notes to inform family members of their loved one’s daily condition. Care plan/Nursing Notes include:
